IAWW 2009 Application Form

Instructions: This application form cannot be saved so you should set aside about 20-30 minutes to fill it out completely. There is a 600 word statement required at the end that you can type in a word processing program like Microsoft Word, then cut and paste into our form. Once you have completed the form, we must receive your US$195 application deposit before your application will be evaluated. You will be contacted when your application materials are complete. Refund and Cancellation Policy: For cancellations received on or before June 1st, 2009, all fees except for the US$195 deposit will be refunded. Participants who cancel after June 1st, 2009, will be charged a US$500 cancellation fee. Once the program begins, no refunds will be made, regardless of whether a participant attends the program. The effective date of cancellation will be the date the written notice of cancellation is received by the program director.
